What to bring

In order to facilitate your check-in when you arrive, please make sure you have the following:

  • a photo id

  • all insurance cards

  • a credit card to put on file

  • a list of medications

  • completed New Patient forms

Practice Policies


Payment Policy

Starting January 1, 2025, we will require that you provide a credit card on file with our office. Payments will be run through our HIPAA-compliant credit card processor, InstaMed. The credit card on file will be used to pay account balances after your insurance has processed. Once your insurance has processed your claims, they will send an Explanation of Benefits (EOB) to both you and our office showing your total patient responsibility. If you disagree with the patient responsibility, please contact your insurance company immediately to dispute their determination.

If you choose not to provide a credit card on file, we will require you to pay for the visit upfront and then reimburse any funds remaining after your insurance has made their payment determination.

Cancellation Policy

Your appointments are very important to us. Time allocated for an appointment is reserved especially for you. We do understand that sometimes schedule adjustments are necessary; therefore, we respectfully request at least 24-hour notice for adjustments to your appointments and for cancellations. As a courtesy, we provide an appointment reminder via telephone or text. However, if we do not speak directly to you it is still your responsibility to be present for your appointment.

Covid-19 Policies

The health and safety of our patients and staff remains our top priority. We are taking every precaution to minimize risk during your appointment. These measures are in line with the current guidelines for medical facilities in the states of Kansas and Missouri.
