What to bring
In order to facilitate your check-in when you arrive, please make sure you have the following:
a photo id
all insurance cards
a list of medications
completed New Patient forms

The health and safety of our patients and staff remains our top priority. We are taking every precaution to minimize risk during your appointment. These measures are in line with the current guidelines for medical facilities in the states of Kansas and Missouri.
If you are not vaccinated we require that you wear a mask. If you are vaccinated and prefer to wear a mask, please do so. Upon arrival to our office, patients will be taken to the examination room, where paperwork will be completed. Each room is thoroughly sanitized between patients.

Cancellation Policy
Your appointments are very important to us. Time allocated for an appointment is reserved especially for you.
We do understand that sometimes schedule adjustments are necessary; therefore, we respectfully request at least 24-hour notice for adjustments to your appointments and for cancellations. As a courtesy, we provide an appointment reminder via telephone or text. However, if we do not speak directly to you it is still your responsibility to be present for your appointment.
Payment Policy
Please be prepared to pay any co-pays or unpaid balances at the time of your appointment. Partners in Rehab Medicine submits billing to your insurance provider, however you are responsible for any co-pays, co-insurance and/or deductibles not covered by insurance.
If you have a high-deductible insurance plan, you may be asked to make an initial payment at the time of service. We will then submit billing to your insurance provider and send you a statement for any balance remaining after your initial payment and insurance has processed.
